Questionnaire on The role of leadership and organisational culture on shaping employee performance

T​‌‍‌‍‍‍‌‌‌‌‌‍‌‍‍‌‍‍‍​his is just a questionnaire for now. It needs to consist of both quantitative (closed) and qualitative (open ended) questions. A number of questions have been agreed with supervisor and it is anything between 10-15 with at least 3 of them​‌‍‌‍‍‍‌‌‌‌‌‍‌‍‍‌‍‍‍​ being qualitative.
